  Broker Application Form
Not yet an approved SF Partners approved broker? The relationship we maintain with our brokers is very important to us and this application form spells out what you can expect from us as your commercial mortgage lender as well as your responsibilities as the mortgage broker.

 1003 Authorization
Uniform Residential Loan Application

Borrower Authorization Form
Borrower’s authorization to allow us to verify funds

 Borrower Signature Authorization
This is the written authorization from the borrower that allows us to gather information about them necessary to process the loan. 

  Loss Payee Information
Necessary information in the event of a loss involving the property.

Mortgagee Clause for Title
Information necessary to notify insurance agent about the change in mortgage holders on your property so that the old mortgage holder is removed and the new one added to protect their interest in your property

 Rent Roll Form
Documents occupancy and rental income of the rental property (not required for owner occupied property).

 Loan Request Form
This form, along with the loan application (1003) and a tri-merged credit report is all we need to provide you with a pre-approval within 24 hours
